so hi I'm Cecilia Hart I'm a community learning disability nurse I support people of learning disabilities in the community I've been using choice since I joined the team March last year I use it regularly on a daily basis particular scenario is when I went out and met somebody for the first time the carer supporting that person struggle to actually give me all the information that I required to be able to inform my assessment in return I used try to try and inform the in from inform what I didn't know from that I actually realized that that person had actually been seen by a neurology was actively open to neurology I returned to that placement informed them at this they were actually unaware and this actually informed their own support plans and what they needed to follow-up to ensure that person had everything they needed
